This application must be returned by August 30, 2006

Once you receive your ice assignment (about mid-September with your full cost noted), you will have until October 11th to do one of the following:

1. pay amount in full for the year (5% discount) or
2. two post-dated cheques dated Oct. 11, 2006 and Jan. 3, 2007 (3% discount) or
3. cheques dated Oct. 11, Dec. 1, Jan. 1, Feb. 1, Mar. 1 (no discount) in the amount of your total cost divided by 5 for each cheque.
